Leflunomid tedavisine bağlı onikomikoz benzeri tırnak değişikliği

Abstract

Leflunomid, romatoid artrit tedavisinde kullanılan antiproliferatif, antiinflamatuar ve immünmodülatör etkili oral olarak alınan yeni bir ilaçtır. Romatoid artrit tedavisinde hastalık modifiye edici antiromatizmal ilaç grubunda yer almaktadır. Romatoid artrit dışında birçok otoimmün hastalıkta da kullanıldığı bildirilmektedir. İlaca bağlı yan etkiler genellikle hafif, doz bağımlı ve geri dönüşlü olup, leflunomid güvenli bir immünsüpresandır. İlaç kesmeyi gerektiren en sık yan etkiler, gastrointestinal yakınmalardır. Literatürde dermatolojik yan etkileri olgu sunumları ile sınırlıdır. Çalışmamızda, romatoid artrit nedeni ile leflunomid kullanan, tedavi sırasında el tırnaklarında proksimal subungual hiperkeratoz, onikoliz, sarı-kahverengi renk değişikliği ve periungual eritem ile kandidal onikomikoz benzeri tırnak değişikliği olan bir hastayı literatürde daha önce raporlanmadığı için sunuyoruz.

Leflunomide is a novel, effective antiproliferative, anti-inflammatory and immunomodulatory drug orally used to treat rheumatoid arthritis. It is located in the group of disease modifying antirheumatic drugs in the treatment of rheumatoid arthritis. It has been reported to be used in the other certain autoimmune diseases besides rheumatoid arthritis. Leflunomide-related side effects are usually mild, dose-dependent, reversible and therefore it is a safe immunosuppressant. The most common side effects of the drug requiring drug-stopping are gastrointestinal complaints. Dermatological side effects in the literature have been reported as case series. Herein, we would like to present our case with candidal onychomycosis-like nail changes of proximal subungual hyperkeratosis, onycholysis, yellow-brown discoloration and periungual erythema since there is no similar case in the literature.
