Öğe Awareness of prostate cancer diagnosis and management among Turkish males: a cross sectional study from Çorum(Taylor and Francis Ltd, 2019) Sungur, Mustafa; Çalışkan, SelahattinIntroduction: Prostate cancer is the most common malignancy among men in the United States and the second most common cancer in Turkey. The incidence of prostate cancer is increasing in industrialized countries. Aim: The aim of the study was to assess the knowledge about prostate cancer, its diagnosis, and treatment among patients with lower urinary tract symptoms. Methods: This study was performed from January to April 2015 with the patients applied to our clinic. A questionnaire that includes 10 questions was administered to the participants. Results: One hundred fifty-nine participants were included in this study. The participants’ ages were between 40 and 82 with a mean age of 61.5 ± 7.9 years. Patient awareness of prostate biopsy and prostate cancer were 21.37 and 71.06%. The main origin awareness of PSA testing is family and friends. On the other hand, if the doctor advises acout prostate biopsy, 47.16% of the patients would accept and 11.31% of them would refuse this invasive procedure. Conclusion: Prostate cancer is one of the important health-related problem among men in the world. Additional researches are needed to investigate the knowledge of prostate cancer among men and the Ministry of Health may take preventive methods to increase the cancer knowledge level of people. © 2019, © 2019 Informa UK Limited, trading as Taylor & Francis Group.Öğe Bibliometric Analysis of the Bladder Cancer Publications Between 1975-2018(2020) Sungur, Mustafa; Çalışkan, Selahattin; Lokman, Utku; Kaya, Coşkun; Şentürk, Aykut Buğra; Aydın, CemilBackground: Bladder cancer is one of the major health problems studied over the years. Emerging technologies on follow-up and treatment of bladder cancer increased scientific literature worldwide. Despite this, there is limited data about the bibliometric analysis of bladder cancer. Materials and Methods: The bibliometric analysis of bladder cancer publications between 1975-2018 was performed by using the Thomson Reuters Web of Science database. The VOSviewer software tool was used for arranging and setting bibliometric networks. Results: The United States of America ranked first in this field with 7963 publications and 28.3% of the world generation, followed by the Peoples Republic of China and Germany. The University of Texas System is the leading institute in published documents with 1081 papers and 3.8% of the total literature. 15.9% of the papers were printed in The Journal of Urology with 4483 manuscripts, followed by European Urology (n=867, 3.0%) and Urology (n=810, 2.8%). Conclusions: There are relatively few articles on bibliometric analysis of urologic disorders and bladder cancer. Advanced evaluation parameters have to be defined for a more confident assessment of scientific work. Bibliometric analyses can guide young clinicians in investigating new studies.Öğe Bladder leiomyoma(College of Physicians and Surgeons Pakistan, 2017) Çalışkan, Selahattin; Sungur, MustafaLeiomyoma of the bladder is a very rare disorder that accounts for 0.43% of all bladder neoplasms. Although the pathophysiology of the bladder leiomyoma is unknown, there are some theories in it. The patients can be asymptomatic; and clinical symptoms, when present, are associated with the tumor size and location. Imaging techniques such as ultrasonography, intravenous urography, computed tomography, and magnetic resonance imaging are helpful but definitive diagnosis is made by histopathological examination. Surgical resection of tumor with transurethral, open, laparoscopic and robotic approaches is the main treatment. We present a case of leiomyoma of the bladder in an adult male patient.Öğe Bladder stone secondary to prolene suture after gynecologic surgery(NLM (Medline), 2018) Sungur, Mustafa; Çalışkan, Selahattin; Lokman, Utku; Savcı, Ünsal; Şahin, MustafaBladder stones are usually seen among patients with bladder outlet obstruction, especially in men over the age of 50 years. There are several risk factors for bladder stones, including urinary tract infections, abnormal urinary tract system anatomy, and presence of foreign bodies. In addition, migrating intrauterine contraceptive devices through the wall of the urinary bladder and foreign bodies such as surgical sutures may act as a nidus for developing stone formation. Here in, we report a case of bladder stone associated with surgical non-absorbable suture, used for gynecologic surgery 4 years ago, and treated endoscopically. The aim of this paper was to know gynecological association and the management of bladder stones. The patients who had a history of pelvic surgery previously and present with lower urinary tract symptoms such as dysuria, voiding difficulties, weak micturition and hematuria should be evaluated for the foreign body-associated disorders.Öğe Can Seasons be an Etiologic Factor for Monosymptomatic Nocturnal Enuresis?(Cureus Inc, 2018) Şentürk, Aykut Buğra; Aydın, Cemil; Ekici, Musa; Çakıroğlu, Basri; Sungur, Mustafa; Baykam, MuratIntroduction Primary nocturnal enuresis is the most frequent urinary system complaint among pediatric patients. Material and Methods Data compiled from 5,500 children, aged between five to 16 years, diagnosed with enuresis during the period from January 2010 to December 2015 were analyzed. The inclusion criteria were having a diagnosis of monosymptomatic nocturnal enuresis, a birth date known for certain, and complete family history taken. A total of 3,547 children met the inclusion criteria and were included in the study. The study was performed by retrospective analyses. Results Analysis of the results revealed a statistically significant difference among the rates of enuresis with respect to months and seasons (p < 0.001). In our study, we retrospectively reevaluated 3,500 patients for their birth dates and determined a statistically significant difference in the rates of nocturnal enuresis with respect to seasons (p < 0.001). Conclusion As a result of this study, we determined that monosymptomatic nocturnal enuresis in children is more frequent, particularly in those born during the summer months.Öğe Combination or Alone? Which one is the Best in Premature Ejaculation Treatment?(2020) Şentürk, Aykut Buğra; Yılmaz, Ali Haydar; Çakıroğlu, Basri; Yaytokgil, Muhammet; Sungur, Mustafa; Aykan, Serdar; Ekici, MusaIntroduction: To compare the effectiveness of tadalafil alone and in combination with dapoxetine in patients with premature ejaculation. Methods: In this study, 120 patients treated at the urology clinic for premature ejaculation were evaluated prospectively. IELT and IIEF forms of patients were filled before this study, and their urological examinations were performed. Patients that were being treated were divided into two groups. Patients in group 1 received only 5 mg tadalafil, whereas patients in group 2 received a combination of dapoxetine 30 mg and tadalafil 5 mg. Results: After the treatment, group 1 mean IEFF scores were 25.12±2.49, and mean IELT scores were 7.35±4.37 seconds. There was a statistically significant increase in both parameters (p<0.001). After the treatment, group 2 mean IEFF scores were 27.37±2.46, and mean IELT scores were 9.07±4.20 seconds. There was a statistically significant increase in both parameters (p<0.001). Discussion and Conclusion: Use of dapoxetine in combination with tadalafil instead of using tadalafil alone in the treatment of premature ejaculation will increase treatment success.Öğe Comparison of the international prostate and visual prostate symptom scores in Turkish patients(NLM (Medline), 2018) Alkan, S.; Sungur, MustafaINTRODUCTION: Benign prostate hyperplasia is one of the most common disease in elderly men. The patients usually present lower urinary tract symptoms including frequency, urgency, nocturia and interittancy. The prostate symptom scores are usually used for daily urology practice. We aimed to compare the International Prostate Symptom Score and Visual Prostate Symptom Score in Turkish patients. METHOD: s. The study protocol was conducted from from February to August 2017. The patients presenting with lower urinary tract symptoms over the age of 45 years were included. The age, prostate specific antigen level, education level and symtom scores were recorded. The patients with history of neurological disease, prostatic surgery, radiotherapy and any malignancy were excluded. RESULTS: There were 81 patients in the current study. The mean age and PSA level was 58.86+/-7.39 years and 1,94+/-1,77 ng/ml. Most of the patients (54.32%) had primary level of education and the patients who were graduated from university (19.75%) followed. The secondary and high school level was 12.34% and 13.58% respectively. The fully complete rate of the symptom scores 37% for IPSS and 9.8% for VPSS and 81% for CSS (p<0.05). In the patients with fully complete; there was no significant difference for patients education level and age. CONCLUSIONS: The IPSS and VPSS are established symtom scores for patients with benign prostate hyperplasia. The difficulties to understand and translate problems of these questionnaires; new, mother language originated symptom score is needed for Turkish patients.Öğe Effect of venous reflux time on testosterone and semen parameters of infertile males after microscopic varicocelectomy(Wiley, 2020) Şentürk, Aykut Buğra; Çakıroğlu, Basri; Yaytokgil, Muhammet; Aydın, Cemil; Sungur, Mustafa; Akkoç, AliThis study aimed to search whether there is a link between reflux flow and hormonal profile. Data of 250 patients were retrospectively investigated. Pre-operative and 6th month semen analyses of these patients' results were compared with pre-operative and post-operative 6th month serum levels of testosterone. Based on the venous flow on pre-operative Doppler ultrasonography, patients were divided into two groups. Patients with a venous flow <= 4 s were assigned to Group 1 and those with a venous flow >4 s to Group 2. Venous reflux time cut-off was calculated as 4 s using ROC curve according to the increase in testosterone level. When venous reflux time was selected as approximately 4.0 s, ROC had 79.8% sensitivity and 72.8 specificity. The probability of a post-operative increase in testosterone is high in patients with a venous reflux time longer than 4 s.Öğe Fibrinogen and D-dimer levels in prostate cancer: Preliminary results(Elsevier B.V., 2017) Çalışkan, Selahattin; Sungur, MustafaBackground Prostate cancer is one of the most common malignancy in men. The main risk factors for coagulation activation and thrombosis are malignancy and older age. The thrombosis risk may be associated with increased level of coagulation markers such as fibrinogen and D-dimer. The aim of this study is evaluate the relationship between coagulation markers and prostate cancer. Methods This prospective study includes the patients who underwent transrectal ultrasound guided prostate biopsy and prostate surgery was performed between January 2015 and January 2016. Plasma prostate specific antigen (PSA), free PSA (fPSA), percentage fPSA, D-dimer and fibrinogen levels were measured before the procedures. The patients were divided into two groups according to the pathology results. The patients with benign prostate hyperplasia were in group 1 and the patients with prostate cancer were in group 2. Results There were 76 patients in the current study. There were 53 patients in group 1 and 23 patients in group 2. The mean age of the patients, PSA, fPSA, fibrinogen and D-dimer levels was 65.33 ± 7.47 years, 8.21 ± 4.59, 1.41 ± 0.74 ng/ml, 309.75 ± 80.46 mg/dl, 0.42 ± 0.39 ug/ml in group 1. In group 2; the mean age of the patients, PSA, fPSA, fibrinogen and D-dimer levels was 66.08 ± 6.7 years, 145.69 ± 509.35, 7.32 ± 15 ng/ml, 312.16 ± 69.48 mg/dl, 1.09 ± 2.11 ug/ml. The prostate biopsy and transurethral surgery were performed in 64(%84.21) and 12(%15.79) patients. Conclusion The present study demonstrated that plasma D-dimer level was higher in patients with prostate cancer. Further studies that include large number of patients are needed to define the relationship between prostate cancer and coagulation disorder. © 2017Öğe Fournier’s gangrene: Review of 36 cases(2019) Çalışkan, Selahattin; Özsoy, Emrah; Sungur, Mustafa; Gözdaş, Hasan TahsinBACKGROUND: Fournier’s gangrene (FG) is a very rare disease in daily urological practice. Despite medical improvements, mortality in FG is still high. Early diagnosis is very important to reduce additional instrumentations and mortality. In this study, we aimed to present the characteristics of the patients with Fournier’s gangrene followed in two centers during ten years period. METHODS: The medical records of patients with FG were reviewed retrospectively. The patient characteristics, causative pathogens, laboratory findings and treatment modalities were evaluated. RESULTS: A total of 36 FG cases admitted between January 2008 and February 2018 were included in this study, consisting of 35 male patients, and one female patient with a mean age of 59.27±12.91 years. The mean duration of hospital stay was 19±10.44 days. The most common predisposing factor was diabetes mellitus, which was found in 28 patients. Malignancy was detected in three patients; prostate cancer in two patients and chronic lymphoblastic leukemia in one patient. Two patients had liver cirrhosis, and one patient had Behcet’s disease and psoriasis. The microbiological agent was isolated from a wound culture in nine patients. After urgent surgical debridement, daily dressing with nitrofurazone (Furacin) was done. Additional debridement was conducted when necessary. Orchiectomy was performed in 10 patients; two of them underwent bilateral orchiectomy. One patient died because of sepsis on the seventh day of hospital admission. CONCLUSION: FG is a life-threatening urological emergency with a high mortality rate. Treatment with broad-spectrum antibiotics and urgent surgery is pivotal for the prevention of mortalityÖğe HALP score and albumin levels in men with prostate cancer and benign prostate hyperplasia(Wiley, 2021) Kaya, Coşkun; Çalışkan, Selahattin; Sungur, Mustafa; Aydın, CemilAims To evaluate the diagnostic significance of the novel index combining preoperative haemoglobin, albumin levels, lymphocyte and platelet counts (HALP) in prostate cancer (PCa) patients. Methods Between 1 January 2015 to 31 December 2018 at the Hitit University Erol Olcok Education and Research Hospital 225 patients who had undergone transrectal ultrasound guided prostate biopsy or prostate surgery were analysed retrospectively. A total of patients, 155 had benign prostate hyperplasia (group 1) and 70 had PCa (group 2). The preoperative serum levels of haemoglobin, albumin, lymphocyte counts and platelet counts were recorded. The HALP scores and the sub-parameters of this index for each of the two groups were compared. Results The total Prostate-Specific Antigen (PSA), albumin and lymphocyte differences between the groups were statistically significant (P = 0.0002, P = 0.0001, P = 0.005). The median value of HALP scores in Group 1 and 2 were 49.43 and 51.2, respectively, and this was not statistically significant between groups (P =0 .737). The HALP score had the least Area Under Curve (AUC) value compared with the others (0.514). The AUC of Albumin was larger than PSA for diagnostic efficacy in PCa patients (0.696-0.656). However, albumin levels were statistically significant compared with platelet count and the HALP score (P = 0.0033, P = 0.0068), except PSA and lymphocyte (P = 0.4580, P =0 .1717). Conclusion Further prospective clinical studies that include more patients from multiple centres are needed to show the diagnostic role of the HALP score and its compounds on the patients with PCa.Öğe High grade urothelial carcinoma of renal pelvis presenting as spontaneous urinoma(2018) Sungur, Mustafa; Çalışkan, SelahattinPerirenal urine extravasation, also known as urinoma, occurs usually after renal trauma. Spontaneous urinoma is a very rare pathology in daily urology practice. The patients with spontaneous urinoma usually present with flank pain, and radiologic imagings confirm the diagnosis. Ultrasonography, intravenous urography, and computed tomography are the radiologic modalites for the diagnosis. We report a case of spontaneous urinoma in a patient with renal pelvic tumor.Öğe Hydrocalyx: uncommon complication of percutaneous nephrolithotomy(College of Physicians and Surgeons Pakistan, 2018) Sungur, Mustafa; Çalışkan, Selahattin; Lokman, UtkuThe success rate of percutaneous nephrolithotomy (PNL) is about 90%, but overall complications of this surgery have been reported in up to 83% cases. The complications are graded using Clavien classification system from grade 1 to 5. Grade 1 needs simple treatments without surgical, endoscopic, and radiologic interventions like antiemetics, antipyretics, and analgesics. Grade 5 denotes death of the patient. Calyx neck stricture is grade 3b complication with an incidence of 0.1%. In this study, a case of hydrocalyx, which is very rare complication after PNL, is presented. To the best of the authors' knowledge, there is no hydrocalyx case reported after PNL in the literature. © 2018 College of Physicians and Surgeons Pakistan. All rights reserved.Öğe Impact of prostate specific antigen level on oncological outcomes after open radical prostatectomy(College of Physicians and Surgeons Pakistan, 2019) Sungur, Mustafa; Çalışkan, SelahattinObjective: To compare the impact of prostate specific antigen (PSA) on oncological outcomes in the patients with 4-10 and 10.01-20 ng/ml PSA level. Study Design: Retrospective comparative study. Place and Duration of Study: Hitit University, Çorum Erol Olçok Training and Research Hospital, Çorum, Turkey, between February 2010 and January 2018. Methodology: Patients who underwent open radical prostatectomy in our department were reviewed retrospectively. The patients were divided into two groups according to the PSA level at diagnosis between 4 and 20 ng/ml. The patients with PSA level of 4-10 ng/ml was in group 1 and 10.01-20 ng/ml was in group 2. Preoperative data including age, biopsy Gleason score (GS), PSA level, postoperative pathological reports, T stage, GS, positive surgical margin, upgrading and downgrading, were compared between groups. Results: There were 109 (71.24%) and 44 patients (28.75%) in group 1 and group 2, respectively. The median age and PSA level of the patients was 67 and 64.5 years; 6.12 and 12.45 ng/ml in both groups, respectively. There was no significant difference for age, GS, number of positive biopsy cores except the PSA level (p<0.001). The difference did not reach statistical significance for stage, upgrading, downgrading, positive surgical margin, and prostatectomy GS between groups. Conclusion: Serum high PSA level at the time of the diagnosis was unrelated to final pathology. The proportion of local advanced disease and positive surgical margin was higher in patients with low PSA values than high levels without statistically significance. © 2019 College of Physicians and Surgeons Pakistan. All rights reserved.Öğe In vitro bactericidal effect of Ho:YAG laser and pneumatic lithotripsy on ureteral stones colonized with Escherichia coli and Enterococcus faecalis(Springer Verlag, 2019) Savcı, Ünsal; Sungur, Mustafa; Şahin, Mustafa; Eser, Barış; Çalışkan, SelahattinThe endoscopic treatment of urolithiasis has a high success rate and the complications decreased after the development of lithotripsy techniques. The aim of this study is to investigate the in vitro bactericidal effect of laser and pneumatic lithotripsy on urinary stones colonized with Escherichia coli and Enterococcus faecalis. A total of 40 natural calcium oxalate stones, which were obtained from the patients’ urinary systems with rigid ureteroscopy were used in the study. Surfaces of the stones were colonized with E. coli and E. faecalis strains. The fragmentation of the stones was performed using holmium:yttrium-aluminum-garnet laser (Ho:YAG laser) and pneumatic lithotripters in vitro in Eppendorf tubes filled with liquid. After fragmentation, samples taken from Eppendorf tubes were inoculated on blood and EMB agar. The number of colonies was evaluated after 18–24-hour incubation period. The laser lithotripsy technique reduced the number of colonies by 100% and had bactericidal effect on E. coli and E. faecalis. Pneumatic lithotripsy technique had no bactericidal effect on these strains (0%). In the fifth minute of laser irradiation, the average temperature in the Eppendorf tube was 51–55 °C, and the average temperature in the tenth minute was 54–60 °C. The temperatures did not change in the fifth and tenth minutes with the pneumatic lithotripsy procedure. The present study revealed the bactericidal effect of Ho:YAG laser on E. coli and E. faecalis in vitro. Increased ambient temperature during Ho:YAG laser lithotripsy is thought to play a role in the bactericidal effect. But the question of whether an ideal lithotripter efficiently inactivates or destroys bacteria has still not been answered in urology practice. Ischemia-modified albumin is a marker of oxidative stress and inflammation. Objective: The aim of this study was to compare the levels of ischemia-modified albumin of telogen effluvium patients with healthy controls. Methods: Ninety-one patients diagnosed with telogen effluvium and 35 healthy volunteers were included in the study. Serum ischemia-modified albumin level was determined by a fast-colorimetric method, and albumin cobalt binding test. The results were evaluated statistically. Results: There was no statistically significant difference between the serum albumin values of patient and control groups (p = 0.739). Serum ischemia-modified albumin values were significantly higher in the patients with telogen effluvium than healthy controls (p < 0.001). Study limitations: Body mass index values of the patient and control groups could not be calculated. Conclusions: To the best of the authors' knowledge, this is the first clinical study to investigate the role of oxidative stress in the pathogenesis of telogen effluvium using ischemia-modified albumin as a biomarker. Based on the results of the present study, it can be considered that oxidative stress plays an important role in the pathogenesis of telogen effluvium. There is a need for further studies to support the results of this study, to demonstrate the possible effects of oxidative stress, and to investigate the other oxidative stress markers in the pathogenesis of telogen effluvium. This abnormality is frequently associated with an obstructed and poorly functioning upper pole moiety. Most of the patients are asymptomatic and diagnosed incidentally. The diagnosis and optimal management of duplicated collecting systems are difficult due to high anatomical variability, the degree of obstruction and chronic damage. The clinical presentations are flank pain, hematuria and urolithiasis and may be complicated by urinary tract infections. Multiple studies demonstrated the relationship between obesity and PCa in the recent years.AIM: The present study aimed to investigate the impact of obesity on postoperative oncological results after radical prostatectomy.MATERIALS AND METHODS: A total of 110 patients who were treated radical prostatectomy between January 2011 and April 2016 were analyzed retrospectively. The patients who had information about age, height, weight, biopsy results, PSA level and pathological results were recorded. The patients were classified to three groups according to the BMI; normal (BMI<25 kg/m2), overweight (BMI>25 and <30 kg/m2) and obese (BMI>30 kg/m2).RESULTS: The present study included 101 patients. Of these patients; 26, 57 and 18 patients were in groups respectively. The age at the presentation and PSA level was lower and higher than the other patients. The proportion of locally advanced disease and high grade PCa were the highest in obese patients at prostatectomy specimen examination. The upgrading is significantly associated with obesity.CONCLUSION: This study demonstrated that obese men are younger and had higher PSA concentration at the diagnosis of PCa. High grade PCa, locally advanced disease and upgrading was seen much more in patients with BMI>30 kg/m2 at final pathology.
