    A successful management with electroconvulsive therapy of neuroleptic malignant syndrome due to amisulpride

    Neuroleptic malignant syndrome is a severe complication that mainly occurs based on neuroleptic drug treatment and characterized with some autonomic symptoms, altered mental state, hyperthermia and muscle rigidity. Amisulpride is a unique atypical antipsychotic that selectively blocks D2 and D3 receptors presynaptically in the frontal cortex and atypical antipsychotics are rarely caused neuroleptic malignant syndrome (NMS). This study examine a 20-years old male with the diagnosis of acute psychotic disorder occurred amisulpride-induced NMS and successfully treated with Electroconvulsive Therapy (ECT). We decided to present that case because of the fact that there is only limited number of amisulpride-induced NMS cases reported in the literature. © 2018 by the authors; licensee Modestum Ltd., UK.

    Antidepresan kullanan ve kullanmayan obez kadınlarda bilişsel davranışçı grup terapisinin etkisi

    Amaç: Çalışmamızda obezitesi olan kadınlara uygulanan bilişsel davranışçı grup terapisinin (BDGT) kilo kaybı, depresyon ve anksiyete düzeylerine etkisini incelemeyi amaçladık ve antidepresan ilaç alan obezitesi olan kadınların tedavisinde de bilişsel davranışçı terapinin etkili olabileceğini göstermeyi planladık. Yöntem: Çalışmaya alınma ölçütlerine uyan 28 obez kadın olguya 14 seans BDGT uygulanmıştır. Terapi öncesi ve bitiminde ölçüm ve klinik ölçek uygulaması yapılmıştır. Terapiden altı ay sonra olguların ağırlıkları tekrar kaydedilmiştir. Olgular Sosyodemografik Bilgi Formu, beden kitle indeksi (BKİ) takibi, DSM-IV için Yapılandırılmış Klinik Görüşmesi (SCID-I), Durumluk-Sürekli Kaygı Ölçeği (DKÖ-SKÖ), Beck Depresyon Ölçeği (BDÖ), Kısa Form-36 (SF-36), SCL-90 Belirti Tarama Listesi (SCL-90) ile değerlendirilmiştir. Başlangıca göre terapi sonunda olguların BDÖ (p=0.009), SKÖ (p=0.020), SCL-90 (p=0.001), SF-36 fiziksel işlev (p=0.035), SF-36 genel sağlık (p=0.004), SF-36 sosyal işlev (p=0.034) puanlarında istatistiksel olarak anlamlı fark saptanmıştır. BKİ’lerinin terapi bitimden altı ay sonra yapılan ölçümleri ile terapi sonundaki ölçümleri arasında istatistiksel olarak anlamlı fark olmadığı saptanmıştır. Sonuç: Elde edilen bulgularla uyguladığımız yöntemin işe yaradığı söylenebilir. Buna göre, BDGT BKİ kontrolünde, ruhsal iyilik durumunun sağlanmasında ve yaşam kalitesinin iyileşmesinde obez kişiler için uygun bir tedavi yöntemidir. (Anadolu Psikiyatri Derg 2020; 21(3):269-276)

    Anxiety and depression symptoms, self-esteem and body image among patients with gynecological cancers: a cross-sectional study

    Aim: Gynecological cancers are among the most important health problems of women, with high rates of morbidity and mortality. These patients also have high rates of anxiety and depression symptoms which can affect the quality of life, self-care, self-esteem, body image, adherence, response to treatment and prognosis of the cancer. This study aimed to investigate the anxiety and depression symptoms, self-esteem and body image among patients with gynecological cancers. Material and Method: In this study, anxiety and depression symptoms, self-esteem, body image satisfaction, and the effect of cancer on these parameters were investigated in eighty-one patients with gynaecological cancer who underwent surgery and have been taking chemotherapy currently. After obtaining the informed consent, a socio-demographic data form, Hospital Anxiety and Depression Scale (HADS), Body Image Scale (BIS) and Coopersmith SelfEsteem Scale (CSES) were applied to all participants. Results: HADS-anxiety score was positively correlated with HAD depression score (p<0.001) and negatively correlated with CSES (p?0.01) score. HADS-depression score was negatively correlated with BIS score (p<0.01). HADS-anxiety score predicted the decrease in self-esteem weakly. HADS-depression score predicted the decrease in self-esteem and deterioration in body image as close to statistically significant. Increased symptoms of anxiety and depression as well as declined self-esteem and impaired body image were observed in patients treated for gynecological cancers. Conclusion: In patients with gynecological cancers, the existence of possible anxiety and depression symptoms, impairment in selfesteem and body image should be screened; if detected, the treatment should be performed for the health of the patients.

    Assessing the relationship between antipsychotic drug use and prolidase enzyme activity and oxidative stress in schizophrenia patients: a case-control study

    Background The relationship between proline, its association with oxidative stress, and its connection to schizophrenia is a subject that has not been sufficiently investigated. Objective The aim of this study is to evaluate the possible effects of atypical and combined (typical and atypical) antipsychotic use on serum prolidase enzyme activity (SPEA) and serum oxidative stress parameters, and to assess the relationship between SPEA and oxidative stress in patients with schizophrenia. Methods A total of 57 patients with schizophrenia, of which 34 were using atypical (AAPG) and 23 were using combined (typical and atypical) (CAPG) antipsychotic therapy, and 28 healthy volunteers (control group) were included in this case-control study. Results SPEA levels of AAPG and CAPG were significantly lower than that of control group (P = 0.003). The oxidative stress index (OSI) value of AAPG was significantly higher than the other two groups (P = 0.001). SPEA (<1860 U/l) and OSI (?0.54) could discriminate schizophrenia patients with antipsychotic therapy from control groups (P = 0.001 and P = 0.007, respectively). Lower SPEA levels were associated with antipsychotic use (P = 0.007). Conclusion The SPEA values of patients with schizophrenia on antipsychotics were significantly lower compared to controls. OSI values were significantly higher in atypical antipsychotic recipients compared to those on combined antipsychotics and healthy controls.

    COVID-19 pandemisinin bipolar bozukluk tanılı hastaların uyku kalitesine etkisi

    Amaç: COVID-19 pandemisi sürecinde Bipolar Bozukluk tanılı hastaların uyku kalitesindeki değişimleri ve evde geçirilen süre ile ilişkisini araştırmak, hastaların kilo, sigara ve alkol kullanım miktarlarındaki değişiklikleri değerlendirerek sürecin hastalar üzerine etkilerini sapta?mak amaçlanmıştır. Yöntem: Toplum Ruh Sağlığı Merkezi’nde (TRSM) Bipolar Bozukluk tanısı ile en az 1 yıldır düzenli olarak takip edilmekte olan hastaların dosyaları tarandı. Çalışma kriterlerini karşılayan, dosyasında; pandemi sebepli sosyal izolasyon tedbirleri başlamadan önceki 3 ay içerisinde ve bu tarihten en az 2 ay sonrasında uygulanmış olan Pittsburg Uyku Kalitesi İndeksi (PUKİ), Young Mani Derecelendirme Ölçeği (YMDÖ), Hamilton Depresyon Derecelendirme Ölçeği (HAM-D) verileri eksiksiz olan hastalar çalışma kapsamına alındı. Bulgular: Çalışmaya 18-60 yaş aralığında 42 hasta dahil edildi. PUKİ’nin alt boyutlarından “uyku süresi” ve “alışılmış uyku etkinliği” dışındakilerin ve genel uyku kalitesinin pandemi sürecinde anlamlı olarak bozulduğu ancak bu bozulmanın evde kalınan gün sayısı ile ilişkili olmadığı bulundu. Hastalarımızın %52,4’ünde kilo artışı olduğu, sigara kullananların %7,1’inin sigara kullanımını artırdığı, depresyon ve mani derecelendirme ölçek puanlarında anlamlı değişiklik olmadığı saptandı. Sonuç: Remisyon döneminde olmalarına rağmen, hastaların uyku kaliteleri olumsuz etkilenmiştir. Elde ettiğimiz veri?ler TRSM hizmetlerinin önemine işaret etmektedir.

    Dream anxiety in hemodialysis and peritoneal dialysis patients

    Introduction: Sleep disorders are frequently reported in hemodialysis and peritoneal dialysis patients, but there are no studies investigating the dreams of these patients. This study was an examination of differences in the level of dream anxiety between hemodialysis patients and peritoneal dialysis patients. A comparison of depression and anxiety symptoms, sleep quality, and sleepiness between these 2 groups was also performed.Methods: In this observational, cross-sectional study, 49 hemodialysis patients and 29 peritoneal dialysis patients treated at an outpatient nephrology clinic were enrolled. A sociodemographic data collection form, the Van Dream Anxiety Scale, the Pittsburgh Sleep Quality Index, the Insomnia Severity Index, and the Beck Depression and Anxiety Inventories were administered, and hemoglobin level and fractional clearance of body water by dialysis (Kt/V urea) were measured.Results: There were no significant differences between the groups in terms of dream anxiety (p=0.517), depression (p=0.889), sleep quality (p=0.221), insomnia severity (p=0.152) or hemoglobin level (p=0.505).Discussion and Conclusion: We did not find any significant difference in the level of depression, anxiety, sleep quality, or insomnia severity between hemodialysis and peritoneal dialysis patients in our study. Similarly, the level of dream anxiety was consistent with these findings.

    Dream anxiety in renal transplant recipients

    Objective: Although low quality of sleep has been reported in kidney transplant patients with functioning allografts, there are no previous studies investigating the dreams of these patients. We aimed to investigate the differences in dream anxiety level between renal transplant patients and healthy control subjects. We also planned to compare depression and anxiety symptoms, sleep quality and sleepiness level between these two groups. Methods: Twenty-two living-donor renal transplant recipients followed at an outpatient nephrology clinic and 22 healthy controls were enrolled in this observational cross-sectional study. Sociodemographic Data Collection Form, and the Van Dream Anxiety Scale (VDAS), the Pittsburg Sleep Quality Index (PSQI), the Insomnia Severity Index (ISI), Beck Depression and Anxiety Inventories were used for the assessment of the necessary features. Hemoglobin (Hb), blood urea nitrogen (BUN), creatinine (Cr) and glucose levels were measured. Results: There were no significant differences between the groups in terms of dream anxiety (p = 0.45), depression (p = 0.76), sleep quality (p = 0.8), insomnia severity (p = 0.08) and Hb (p = 0.11) and glucose levels (p = 0.14). Although, BUN (p = 0.00) and creatinine (p = 0.00) levels differed significantly between the two groups, both parameters were found to be within their normal range. Conclusions: In our study, chronic renal failure patients with a successful kidney transplant were found to be able to completely return to normal in terms of metabolic parameters, sleep quality and mood. Similar levels of dream anxiety are also consistent with these findings. © 2015 Informa Healthcare USA, Inc. All rights reserved.

    Evaluating a-synuclein, dopamine and serotonin concentrations in the blood of patients with heroin use disorder: Baseline and post-treatment comparisons

    Background. Oligomerization of a-synuclein is induced by serotonin aldehyde, and impulsive behaviour and substance use disorders may be associated with a-synuclein, dopamine and serotonin. Aim: We aimed to compare blood levels of a-synuclein, dopamine and serotonin in patients with heroin use disorder (HUD) and healthy individuals and to assess the possible effects of HUD treatment on the blood levels of these impulsivity-associated markers. Methods: 17 patients with HUD and 23 healthy volunteers were included in this study. All participants' baseline a-synuclein, 5-hydroxytryptamine (5-HT), dopamine, dopamine beta-hydroxylase (D.H), 5-hydroxy indole acetic acid (5-HIAA), homovanillic acid (HVA), and heat shock protein-70 (HSP-70) blood levels were measured. In patients with HUD, levels of these markers were measured after three weeks of buprenorphine treatment. Results: While the pre-and post-treatment HVA levels of the patient group were significantly higher (p=0.036), the pre-and post-treatment D.H (p<0.001), 5-HT (p<0.001) and dopamine (p<0.001) levels were significantly lower compared to controls. The patient group's post-treatment a-synuclein (p=0.044) levels were significantly higher than controls. With a cut-off value of <2000 ng/mL, DBH levels had a sensitivity of 94.1% and a specificity of 100.0% to detect patients with HUD. With a <251 pg/mL cut-off value, dopamine had 94.1% sensitivity and 82.6% specificity in distinguishing patients with HUD from controls. Conclusions: Dopamine and D.H may be helpful markers in diagnosing HUD. Understanding the influence of heroin use on these markers and pathways may enable their utilisation in assessing impulsive behaviour towards heroin.

    Evolution of Telepsychiatry: Scientometric Analysis of Telepsychiatry Publications Between 1986 and 2019

    Background: Although telepsychiatry has a long history, medical literature lacks a scientometric study evaluating telepsychiatry publications. The purpose of this study was to perform a holistic analysis of telepsychiatry articles published between 1986 and 2019. Methods: We used the telepsychiatry keyword for our search and included all documents indexed in Web of Science (Clarivate Analytics, USA) Core Collection between 1986 and 2019, revealing a total of 1,020 articles, of which only 224 were open access. Results: The peak year for publication was 2015, with 96 articles. The United States ranked first with 601 documents followed by Australia and Canada. The University of California and University of Washington were the most productive institutions and, again, 8 of the 10 leading institutions were from the United States. The peak year for citations was 2019, with a total of 2,080 records. Discussion: We believe that systematic approaches are needed to reveal the positive and negative features of telepsychiatry practice, especially from countries where this method is widely utilized, to elucidate the need for telepsychiatry in other countries/regions and to determine how its use can be increased in regions with limited access to health care workers. Conclusion: Although scientific interest in telepsychiatry appears to have increased almost every year since 1986, it has been observed that this interest is still concentrated in certain countries, such as the United States, Australia, and Canada, indicating that telepsychiatry may not have gained use in other countries.

    Investigation of the Relationship of Serotonin, Dopamine and Their Metabolites with ? – Synuclein in Obsessive Compulsive Disorder

    Objective: Obsessive compulsive disorder (OCD) is a mental disorder characterized by obsessions and/or compulsions. Impulsive features play a role in the etiology, symptomatology and clinical presentation of OCD. ?-synuclein which plays a role in dopaminergic and serotonergic neurotransmission has been shown to be effective in impulse control. The aim of this study was to examine the levels of ?-synuclein, serotonin (5-hydroxytryptamine; 5-HT), dopamine, dopamine ?-hydroxylase (DBH), 5-hydroxyindolacetic acid (5-HIAA), homovanillic acid (HVA) and heat shock protein 70 (HSP70) in OCD. Methods: Twenty-six OCD patients and age-gender matched 23 healthy volunteers were included in this study. Biochemical parameters were analyzed by enzyme-linked immunosorbent assay method. Patients were evaluated with Barratt Impulsivity Scale (BIS) and Dimensional Obsession Compulsion Scale (DOCS). Results: Compared with the controls, significant reduction of 5-HT, dopamine and DBH levels while significant increase of ?-synuclein, HVA and HSP70 levels was found in patients with OCD. Moreover, ?-synuclein levels were significantly negatively correlated with 5-HT, dopamine and DBH, while significantly positively correlated with HVA, 5-HIAA and HSP70. HTR and dopamine levels were found to be associated with the subscale of BIS. Dopamine and DBH levels were found to be associated with the subscale of DOCS. Conclusion: It may be suggested that ?-synuclein is associated with dopaminergic and serotinergic pathways, and may provide important contributions for elucidating the etiology of OCD. It can be suggested that the effect of dopamine is more than 5-HT, based on the fact that not only dopamine but also DBH is correlated with OCD symptoms

    Kardiyovasküler cerrahi yoğun bakım birimlerinde hasta yakınının gereksinimleri: Anksiyete ve depresyon ile ilişkisi

    Objective: In this study, we primarily aimed to determine “needs” and “anxiety and depression symptom levels” among families of patients who were followed up in cardiovascular surgery intensive care unit (ICU). Furthermore, we sought to investigate the association of anxiety and depression with these needs. Material and Methods: This descriptive study was conducted with relatives of 99 patients who were hospitalized for minimum 24 and maximum 72 hours in cardiovascular surgery ICU. Hospital Anxiety and Depression Scale and Critical Care Family Need Inventory were applied. Percentages and averages were used in descriptive statistics. Moreover, t-test and One Way ANOVA tests were used for comparisons between the gender and relationship groups. Pearson’s two-way correlation analysis was used to determine the association between scores obtained from above mentioned scales. Results: The “anxiety” and “depression” subscale scores of female relatives were higher than those of male relatives. The “information” need scores of spouses were higher than those of other family members, and depression level scores of spouses were higher than those of non-consanguineous relatives. Moreover, “proximity/assurance” and “anxiety and depression” subscale scores were related. Conclusion: Patients’ relatives show high anxiety and depression symptoms and they strongly need proximity and assurance. Meeting these requirements may reduce symptoms of anxiety and depression. © 2018 by Turkish Society of Medical and Surgical Intensive Care Medicine

    Preoperative anxiety may increase postcesarean delivery pain and analgesic consumption

    BACKGROUND:The aim of this study was to reveal the relationship between high level of prenatal anxiety and postoperative pain and/or analgesic consumption in women undergoing elective cesarean delivery under spinal anesthesia. METHODS:Eighty women, aged between 18-45 years with minimum 37 week- gestation and received spinal anesthesia during elective cesarean delivery, were included into this observational cohort study. Prenatal anxiety was measured with state anxiety inventory, trait anxiety inventory and somatosensory amplification scale. Visual Analogue Scale (VAS) was used to quantify postoperative pain. Amount of analgesic consumed was recorded at 6th, 12th and 18th postoperative hours. RESULTS:State Anxiety Score was above the threshold level (>45) in 18 women (22.5%). No difference was found between women with and without high state anxiety scores except for significantly higher BMI values in high-score group (P=0.07). In multivariate analysis, high BMI at pregnancy (OR: 1.2, 95% CI; 1.0-1.5, P=0.02) and high State Anxiety Score (OR: 1.1, 95% CI; 1.0-1.2, P=0.01) emerged as independent predictors of higher mean pain scores (VAS >4 cm) within 18 hours after cesarean delivery. Also, high State Anxiety Score was found to be independently associated with higher pethidine consumption after cesarean delivery (OR: 1.1, 95% CI; 1.0-1.2, P=0.006). CONCLUSIONS:State anxiety has a negative effect on postcesarean pain whereas trait anxiety does not seem to produce such effect. The effect seems to be more profound in overweight women. Detection of anxiety level before elective cesarean delivery and therapeutic approach to pregnant women may be useful for postoperative pain control.

    Psikoz olgularında tedavi uyumuna yönelik bir terapi programı uygulanması

    Tedaviye Uyum Terapisi(TUT) literatürde “anlaşılan tedavi etkinliği ve ilaç kullanma motivasyonu” olarak tanımlanmaktadır. Tıbbi tedaviye uyumu arttırmayı amaçlayan, hasta merkezli bir girişimdir. Bireysel seanslar ile uygulanır. Tedaviye uyumu etkileyen bir dizi karmaşık faktörü hedef alır. Hastaların tedaviye yönelik inançlarını düzenleyen motivasyonel ve bilişsel davranışçı yaklaşımlar ile psikoeğitim birlikteliği kullanır. Bu araştırma, TUT’ninpsikoz tanılı hastaların tedaviye uyumlarına etkisini incelemek amacıyla yapıldı. Araştırmada 26 hasta olgu ve 24 hasta kontrol grubunda yer aldı. Kısa Psikiyatrik Değerlendirme Ölçeği ve Tıbbi Tedaviye Uyum Ölçeği kullanıldı. Gruplar arasında yaş ve cinsiyet açısından istatistiksel olarak anlamlı bir fark yoktu. TUT programının kısa dönemde hastalık semptomları ve tedavi uyumu üzerinde iyileştirici etkisinin olduğu tespit edilmiştir. Uzun dönemde ise bu etkinin azaldığı bulunmuştur. Bu bulgu TUT programının psikoz olgularında tedavi uyumunu arttırmaya katkıda bulunduğunu göstermektedir. Araştırma sonuçlarına göre terapi programının en geç altı ayda bir tekrarlanması önerilebilir.

    Reliability and Validity of a Turkish Version of the Acceptance and Action Diabetes Questionnaire

    Objective The aim of this study is to perform validity and reliability examination of the Turkish form of Acceptance and Action Diabetes Questionnaire, and to investigate whether this scale is a measurement tool for evaluation of psychological flexibility levels in a sample of patients with diabetes in Turkey. Methods This study was conducted with 105 patients. Turkish forms of the Beck Depression Inventory (BDI), Problem Areas in Diabetes Questionnaire (PAID), State-Trait Anxiety Inventory (STAI-I and STAI-I I), Audit of Diabetes-Dependent Quality of Life (ADDQoL) and Turkish form of Acceptance and Action Diabetes Questionnaire (TAADQ) were applied. SPSS 20.0 and AMOS was used in statistical analysis. Results 56.12% of the patients were female and the mean of age was 54 (SD=+/- 9.9) years. The mean duration of education was found 7.65 (SD=3.97) years. 74.8% of the patients most of whom (83.3%, n=85) had diabetes mellitus and the mean glycemic control calculated with HbAlc was 8.02 +/- 1.91. According to the final fit indices, we found that the revised and corrected 9-item model was superior over the previous model. Cronbach Alpha coefficient of TAADQ was found as 0.836. Conclusion TAAI)Q is a valid and reliable assessment tool in Turkish population. So TAADQ will be a powerfull tool in assessing psychological flexibility in diabetes patients.

    The effect of perceived parental attitude score on symptoms of bipolar disorder and schizophrenia

    Objectives: We aimed to investigate the effect of perceived parental attitudes on the symptoms of these diseases in patients with schizophrenia and bipolar disorder (BD) and to compare the perceived parental attitudes between these two disorders. Methods: This cross-sectional study was conducted between August 2020 and August 2021 at a university hospital in Turkey. Fifty-two patients with BD and 50 patients with schizophrenia in remission, aged 16-50 years, who met the respective diagnostic criteria defined by the Diagnostic and Statistical Manual of Mental Disorder-5 were included in the study. Results: The mean age of patients with BD was 38.90±10.95 years, while it was 39.08 ± 11.51 years for those with schizophrenia. Females comprised 65.38% (n = 34) of the BD group and 78.00% (n = 39) of the schizophrenia group. Our results showed that the severity of various negative schizophrenia symptoms increased with higher levels of perceived parental libertarian attitude. In addition, the severity of delusion, which is one of the positive symptoms of schizophrenia, was found to increase with lower perceived parental interest. We did not find a significant relationship between the severity of mania and depression symptoms and perceived parental attitudes. Conclusions: In addition to supporting previously reported relationships of various factors and schizophrenia and BD, our results suggest that the increase in the level of liberality of parents has a negative impact on the negative symptoms of schizophrenia. In addition, the decrease in the level of interest of parents towards their children exacerbates delusion symptoms

    The effect of religious belief and forgiveness on coping with diabetes

    We aimed to investigate the relationship of religious beliefs and forgiveness in diabetic patients with various sociodemographic characteristics, emotional problems and glycaemic control. The study comprises 100 patients diagnosed with type 2 DM. We used a data collection form, the Scale of Forgiveness and Religiosity (SFR), Problem Areas in Diabetes Scale (PAID), Beck Depression Inventory (BDI), State-Trait Anxiety Inventory (STAI) and the Audit of Diabetes-Dependent Quality of Life (ADDQoL). We also recorded blood glucose and HbA1c test results. A statistically significant relationship was determined only between the scores of the STAI-I and the religious belief scales (r = 0.198, p = 0.049). A statistically significant negative relationship was determined between the forgiveness scale points and the BDI (r = 0.326, p = 0.001), the STAI-II (r = 0.308, p = 0.002) and PAID (r = 0.313, p = 0.001) and a positive correlation with ADDQoL (r = 0.284, p = 0.004). To conclude, forgiveness by patient himself or others reduced the emotional problems which were experienced related to diabetes by reducing stress levels and could increase quality of life. © 2017, Springer Science+Business Media, LLC.
