Öğe All-seeing-access sheath: A novel fluoroscopy-free placement technique in retrograde intrarenal surgery(College of Physicians and Surgeons Pakistan, 2019) Ekici, Musa; Özgür, Berat Cem; Şentürk, Aykut Buğra; Akdağlı Ekici, Arzu; Aydın, Cemil; Baykam, Mehmet MuratObjective: To evaluate the safety and effectiveness of fluoroscopy-free RIRS (Retrograde intrarenal surgery) method in urolithiasis. Study Design: An observational study. Place and Duration of Study: Department of Urology, Hitit University Corum Training and Research Hospital, Corum, Turkey, from October 2014 and October 2017. Methodology: Preoperative and postoperative data of 62 cases of renal calculi that underwent fluoroscopy-free RIRS procedure by a single surgeon were prospectively evaluated. All manipulations were performed with a novel technique; under direct vision during the operation. Fluoroscopy device was kept at hand in the operating room, but was not used. Results: The mean age was found as 51.73 ±12.63 (22-82) years. Fortyone patients (66.1%) were males and 21 (33.9%) were females. The mean size of stones were 19.29 ±7.64 (10-40) mm. The stone-free rate was calculated as 42 (67.7%) cases in a single session one month after the surgery, and 15 (24.2%) cases after the second session at the postoperative first month. A total of 57/61 (91.9%) patients were stone-free after the second procedure. No major intraoperative and postoperative complications were observed. As for the minor complications, one patient (1.6%) developed hematuria and four patients (6.5%) had fever. Conclusion: The described fluoroscopy-free RIRS procedure can be performed effectively and safely in patients diagnosed with renal calculi, by endourologists. © 2019 Dustri-Verlag Dr. Karl Feistle. All Rights Reserved.Öğe Clinical Manifestations of Overactive Bladder With Migraine as a Comorbidity: A Prospective Cross-Sectional Study(Korean Continence Soc, 2020) Başer, Aykut; Eliaçık, Sinan; Baykam, Mehmet Murat; Uysal Tan, FundaPurpose: The aim of this study was to investigate the clinical manifestations of overactive bladder (OAB) with migraine as a comorbidity and to shed light on possible new treatment. strategies. Methods: This study included patients aged 18 years and older who were admitted to urology and neurology outpatient clinics between March 1, 2019 and March 1, 2020 for OAB and migraine. The study questionnaire contained 3 sections: (1) questions on demographic characteristics, (2) a migraine II) test, and (3) the Overactive Bladder Inquiry Form - V8 (OAB-V8) form. Results: A total of 265 patients participated in the study. The average age of the participants was 39.75 +/- 11.93 years. The patients were divided into 3 groups according to the coexistence of OAB with migraine: group 1, OAB(+)/migraine(+); group 2, OAB(+)/migraine(-); and group 3, OAB(-)/migraine(+). The mean OAB-V8 score was 22.82 +/- 8.15 in group 1 and 25.64 +/- 7.49 in group 2. The mean OAB-V8 score of OAB patients with migraine as a comorbidity was statistically significantly lower than that of OAB patients without migraine (P = 0.015). The median visual analogue scale (VAS) score was 7.11 (range, 2-10) in group 1 and 5.95 (range, 2-10) in group 3. This finding indicates that in patients with migraine, having OAB was associated with significantly higher VAS scores (P <0.001). Conclusions: OAB and migraine may be comorbid conditions coexisting in a single patient. This comorbidity may lead to a lower perception of OAB symptoms in OAB patients or, conversely, to a higher perception of migraine pain. Further studies are needed to elucidate how treatments for each of these diseases can affect the other disease.Öğe Comparison of three most frequently used alpha blocker agents in medical expulsive therapy for distal ureteral calculi, result of a retrospective observational study(PAGEPress, 2018) Şentürk, Aykut Buğra; Aydın, Cemil; Ekici, Musa; Yaytokgil, Muhammet; Akkoç, Ali; Baykam, Mehmet MuratPurpose: In this study, we compared the effects of three agents frequently used in daily life for medical expulsive therapy. Materials and methods: A total of 143 patients meeting the criteria were included in the study. Patients were divided into three homogeneous drug groups which were tamsulosin group (n:48), alfuzosin group (n:47) and silodosin group (n:48). The time of stone expulsion, analgesic needs, side effects of the medicine and endoscopic intervention needs of the patients were recorded. Results: The rate of stone expulsion was 70.8% (n:34) in tamsulosin group, 70.2% (n:33) in alfuzosin group, and 75% (n:36) in silodosin group. No significant difference was observed among the rates of stone expulsion in three groups, and the rates of stone expulsion were similar (p = 0.778). The duration of stone expulsion was significantly different in the groups (p = 0.012): the time of stone expulsion for tamsulosin was 2.33 ± 0.78 days longer than for Silodosin, indicating a significant difference. There was no significant difference between tamsulosin-alfuzosin and silodosin-alfuzosin (respectively p = 0.147, p = 0.925). Conclusions: The results of this study showed that medical expulsive therapy by using alpha blocker agents is safe and efficacious. This option must be kept in mind for patients who do not ask for surgery as the first-step treatment for eligible patients. © 2018 Edizioni Scripta Manent s.n.c. Study Design: Observational study. Place and Duration of Study: Departments of Urology and Neurology, Hitit University Hospital, Turkey, from August 2019 to August 2020. Methodology: A total of 179 patients (92 subjects diagnosed with CM and 87 control healthy volunteers) were included in this study. Demographic descriptive data including age, height, weight, and body mass index (BMI) of all patients were recorded. A 5-question version of the international index of erectile function questionnaire (IIEF-5) was applied to evaluate their sexual functions. Furthermore, a migraine identification test was performed for CM patients for the diagnosis of migraine in accordance with the International Headache Society's (IHS) definition of chronic migraine. Visual analog scale (VAS) scores between 0-10 points were recorded for the qualitative assessment of migraine pain. Study Design: An observational study. Place and Duration of Study: Departments of Urology, University of Health Sciences, Ankara Research and Training Hospital, Ankara, and corum Hitit University, Corum, Turkey from April 2019 to January 2020. Methodology: Patients with kidney stones up to 3 cm in maximal diameter on computarised tomography (CT) scan, were divided into two groups according to age (<60 vs. >= 60 years ). RIRS was performed. Demographics, clinical data, and complications were recorded and compared. Results: Mean age of 78 patients was 52.47 +/- 13.28 years. There were no significant difference in gender, American Society of Anesthesiology (ASA) scores, operation time, number of multiple sessions, success rates of outcome and complication rates between groups. At the end of the first session, stone-free rates were 91.1% vs 87.9% in younger and elderly groups, respectively; while, after the second session, all patients were stone-free in both groups. Fever, hematuria (macroscopic, more than 24 hours), and perirenal hematoma were the observed complications. Postoperative hematocrit levels significantly decreased without any need of transfusion; there were significant differences in the preoperative and postoperative creatinine levels (p=0.002) in the older group. Conclusion: RIRS is a safe and effective method for treating kidney stones up to 3 centimeter diameter in all age groups. Although hemorrhagic events are mostly clinically unremarkable, there is a risk of renal function impairment in older (>= 60 years) patients. Preoperative hydration and postoperative close follow-up is important.Öğe Global Publication Outcomes in Retrograde Intrarenal Surgery and Turkiye’s Effectiveness: A Bibliometric Analysis between 1980 and 2019(2020) Baykam, Mehmet Murat; Ekici, Musa; Demir, EmreObjective: Urinary system stone disease is a very common health problem that affects 2 to 3% of people and causes serious complications when it is not treated. The prevalence of renal stones has been increasing worldwide and surgical methods have changed over time. However, there is no comprehensive bibliometric analysis of the retrograde intrarenal surgery (RIRS) method in the literature. This study aimed to conduct a bibliometric analysis of all articles regarding RIRS published between 1980 and 2019. Materials and Methods: The literature search was performed using only the following keywords: “RIRS” and “flexible ureterorenoscopy/flexible ureteroscopy”, in the “title” part of the search. Using this search method, all the articles published between 1980 and 2019 on this topic were accessed and downloaded from the Web of Science database, and the articles were analyzed using bibliometric methods. Results: Totally, 1378 publications were found as a result of the literature search, of which 619 were articles. We noted that there has been a rapid increase in the number of publications, especially from the year 2011. Turkiye was found to be the most productive country with regard to RIRS. The journals that contributed most to the literature were: Journal of Endourology, Urology, Journal of Urology, Urolithiasis, and World Journal of Urology. Conclusion: One of the notable findings of this study is that Turkiye is among the world countries that produced the highest number of publications with regard to this topic and that the hospitals where this technique is used the most are located in Turkiye.Öğe Is it important to see the coexistent seminal vesicle invasion and extracapsular extension at the radical prostatectomy specimen reports?(2020) Topuz, Sezgin; Özdeş, Cüneyt; Gökkaya, Cevdet Serkan; Tağcı, Süleyman; Aktaş, Binhan Kaan; Bulut, Süleyman; Baykam, Mehmet MuratAim: To evaluate the effect of coexistent extracapsular extension and seminal vesicle invasion on biochemical reccurence free survival rates in patients who had undergone open radical retropubic prostatectomy. Material and Methods: The data of 307 patients with clinically localized prostate cancer who had undergone radical retropubic prostatectomy between January 2000 and May 2019 were evaluated retrospectively. According to extension of tumor on surgical specimens, patients were classified into five groups as; organ-confined disease, extracapsular extension, isolated seminal vesicle invasion, extracapsular extension in addition to seminal vesicle invasion and lymph node involvement. Patient groups were analyzed categorically with regard to biochemical reccurence free survival rates by using Kaplan Meier analysis, log-rank and chi-square tests. The effect of pathological features of surgical specimens on biochemical reccurence was evaluated by using univariate and multivariate Cox regression analysis. Results: There were statistically significant differences on biochemical reccurence free survival rates among the five groups, and pathological stage and biochemical reccurence rates increased correspondingly. Multivariate Cox regression analysis showed that coexistent extracapsular extension and seminal vesicle invasion, and lymph node involvement are the two significant factors that negatively effect the biochemical reccurence free survival rate. Conclusion: Coexistence of extracapsular extension and seminal vesicle invasion is a worse prognostic factor compared to their isolated forms.Öğe Predicting Lamina Propria Invasion in Patients with Nonmuscle-invasive Bladder Cancer: Do RDW and NLR Really Work?(2019) Şentürk, Aykut Buğra; Ekici, Musa; Aydın, Cemil; Baykam, Mehmet Murat; Taş, Tuncay; Arda, Ersan; Çakıroğlu, BasriObjective: To determine the role of preoperative neutrophil-to-lymphocyte ratio (NLR) and red cell distribution width (RDW) in predicting lamina propria invasion in bladder cancer. Materials and Methods: Eighty-eight patients with non-muscle-invasive bladder cancer were evaluated retrospectively. The patients were divided into those with Ta tumors (group 1: n=36) and those with T1 tumors (group 2: n=52). For each patient, white blood cell, neutrophil, and leukocyte counts and RDW values were evaluated. Results: NLR was significantly lower in patients with Ta tumors. In addition, NLR below 3.22 was associated with 80.6% probability of Ta disease. RDW sensitivity in Ta non-muscle-invasive bladder tumors was much higher compared to T1 tumors. RDW below 15.35 was associated with 94.4% probability of Ta disease. Conclusion: NLR and RDW are basic blood parameters that physicians can assess easily. Our results indicate that a combination of NLR and RDW can help clinicians predict lamina propria invasion in non-muscle-invasive bladder tumors.Öğe PSA Seviyesi 4-10 Ng/Ml olan hastalarda alt üriner sistem semptomları Prostat Adenokanser ve Bening Prostat Hiperplazisini ön görebilir mi?(2020) Başer, Aykut; Aydın, Cemil; Çelikörs, Bilal; Yılmaz Başer, Hülya; Baykam, Mehmet Murat; Alkış, OkanAMAÇ: Bu çalışmada PSA seviyesi 4-10 ng/mL olan hastalarda alt üriner sistem semptomalarının prostat adenokanser ve benign prostat hiperplazisini ön görecek bir parametre olup olmadığını araştırmayı amaçladık. GEREÇ VE YÖNTEMLER: 01 Haziran 2019 - 01 Şubat 2020 tarihleri arasında PSA değeri 4-10 ng/ml olan ilk kez TRUS-Bx uygulanan 75 hasta prospektif olarak çalışmaya dahil edildi. Hastaların yaş, vücut kitle indeksi, PSA, Prostat hacmi, PSA dansitesi, uluslararası prostat semptom skorları ve uluslararası erektil fonksiyon skorları kayıt edildi. Patoloji sonucuna göre grup 1 (benign prostat hiperplazisi) ve grup 2 (prostat adenokanser) olarak hastalar 2 gruba ayrılarak belirtilen parametreler ile ilişkisi araştırıldı. BULGULAR: PSA değeri 4-10 ng/ml olan ilk kez TRUS-Bx uygulanan 75 hastanın patoloji sonucunda; 57 (%76) hastada benign prostat hiperplazisi (Grup 1), 18 (%24) hastada prostat adenokanser (Grup 2) saptandı. İncelenen parametrelerden sadece parmakle rektal muayenede pozitiflik (sertlik) bulgusu ve IPSS skor düşüklüğü Grup 2’de, Grup 1’e göre istatistiksel anlamlı farklılık saptandı (sırası ile; p=0.001 ve p=0.022). Yapılan korelasyon analizi ile de bulgular teyit edildi. SONUÇ: PSA 4-10 ng/mL olan hastalar için IPSS skor düşüklüğü, parmakla rektal muayenede pozitiflik (sertlik) olması kanser saptanma ihtimalini arttırmaktadır.Öğe Urethral calculi: a rare cause of acute urinary retention in women(Emergency Medicine Association of Turkey, 2018) Sungur, Mustafa; Baykam, Mehmet Murat; Çalışkan, Selahattin; Lokman, UtkuUrinary retention is defined as the inability to void; frequently the patient is able to void partially, thus can not completely empty the bladder and diagnosed by increased post-void residual urine (PVR) volumes. It is usually seen in men, but a rare condition in women. The etiological factors may be neurological, anatomical, infectious or pharmacological. It can be encountered after gynecological or incontinence surgery. Urethral calculi associated with acute urinary retention in women is a very rare condition, because of the anatomic structure of female urethra. In this study, a case of acute urinary retention due to the urethral calculus in a middle-aged woman was presented. © 2018 The Emergency Medicine Association of Turkey
