Özet
51 yaşındaki kadın hasta 6-7 yıldır artıp azalarak giden boyun yerleşimli lezyonlarla başvurdu. Dermatolojik muayenesinde boyun laterallerinde yoğunlaşan kahverengi milimetrik papülleri ve el tırnaklarında V şeklinde çentiklenmeleri mevcuttu. Histopatolojik inceleme Darier hastalığı ile uyumlu olarak geldi. Darier hastalığı otozomal dominant kalıtımla geçen, deri, tırnaklar ve mukozaların tutulduğu keratotik papüllerle karakterize bir hastalıktır. Darier hastalığının çeşitli klinik varyantları tanımlanmıştır. Lezyonların fleksural yerleşimi çok nadirdir. Olgumuzun lezyonları sadece boyun yerleşimliydi. Bu olguyu Hailey-Hailey hastalığı, Grover hastalığı gibi fleksural yerleşimin daha sık görüldüğü klinik durumlar yanında Darier hastalığının da akılda tutulması gerektiğini vurgulamak amacıyla sunmayı uygun bulduk.
A 51-year-old woman attended with asymptomatic lesions on her neck which had been present for 7 years. On dermatological examination multiple brown milimetric keratotic papules were observed on the lateral sides of neck and V-shaped nicks were observed on the nails of her hand. Histopathological examination of the lesions were consistent with Darier disease. Darier disease is an autosomal dominant skin disorder characterized by keratotic papules, focal loss of adhesion and abnormal keratinization. Several clinical variants of Darier disease have been described. Localized flexural distribution of lesions in Darier disease is uncommon. We report a sporadic case of Darier disease in which the lesions were strictly localized to the neck. Physicians should be aware of such atypical localizations and should consider the diagnosis Darier disease along with more common clinical conditions with flexural localization, such as HaileyHailey disease and Grover disease.
